Home Equity Checklist

To expedite the processing of your home equity loan, please provide the items listed below:

COMPLETE THE APPLICATION thoroughly using the documents and/or information shown below. Name(s) should be listed in same order as credit union account. Failure to do so may result in processing delays.

  • List of all names, addresses and account numbers for all financial institutions and/or creditors.
  • Signed authorization forms enclosed in the Altier CU loan packet.
  • Copy of the existing deed. (Deed of Trust or Warranty Deed)
  • Copy of the front page of homeowner's insurance policy, showing name of agent, phone number and annual premium.
  • Copy of the front page of the flood insurance policy, showing name of agent, phone number and annual premium. (if applicable)
  • Verification of all income you wish us to consider for loan qualification, i.e., copies of Social Security award letters, pension, etc.
  • Verification of amount of yearly property taxes (Residential Notice of Value)
  • Most recent W-2 forms for the last two years on each applicant.
  • One full month most recent pay statements (paycheck stubs) on each applicant.
  • Statements for any loans you want paid off with this loan.
  • If you and your spouse have any income for self-employment or if you and/or your spouse own any other property, most current rental/lease agreement.
  • If you and your spouse have any income for self-employment or if you and/or your spouse own any other property, your personal or business income tax returns 1040's including all schedules for the past two years.
  • If you and your spouse have any income for self-employment or if you and/or your spouse own any other property, a year-to-date Profit and Loss Statement (if Schedule C Business)
  • If Partnership of S-Corporation, two years of completed 1065s, 1120s, K-1s and year-to-date Profit and Loss Statement.
